The doctors and staff here at Harley Eye Clinic are committed to ensuring that their patients receive the best possible care at all times. This policy is to protect patient privacy in compliance with the Privacy Act 1988.
WHAT KINDS OF PERSONAL INFORMATION DO WE COLLECT
For this reason, we will need to collect and store some personal information about you.
- Full Name
- Date of Birth
- Address (Residential and Postal)
- Telephone numbers
- Medicare, DVA, Pension and Health Fund details
- Medical history including allergies, test results and other relevant health information
- Next of Kin / Emergency contact
HOW DO WE COLLECT AND HOLD PERSONAL INFORMATION
We will generally collect personal information from you directly either face to face, via a telephone conversation or the registration form you will fill in on becoming a patient at Harley Eye Clinic. If you are not able to do this then we will collect your information from a person nominated by you.
Your file is paper based and is stored within our on-site filing cabinets in an area accessed only by staff. Some of your health information is stored on our computers, these are protected by password access only with Firewalls and anti-virus software continually updated.
Any information no longer required by the doctors here at Harley Eye Clinic is appropriately destroyed by shredding to protect the patient’s identity and their personal information.
WHY DO WE COLLECT, HOLD AND DISCLOSE PERSONAL INFORMATION
Collecting your personal information allows the doctors here to provide you with the best possible outcomes. This information will only be used in activities relating to your eye care and treatment while you are a patient here at Harley Eye Clinic. This may include correspondence to your GP, Optometrist or another Medical Specialist as well as other allied health professionals involved in your care e.g. radiologists. It may be used for accounting purposes, claiming from Medicare, DVA and your Private Health Fund as necessary.
To comply with our legal obligations, including, but not limited to mandatory reporting, we may be required to disclose your personal information to the relevant authorities.
Your written consent is required before we can disclose any information to third parties.
HOW CAN YOU ACCESS AND CORRECT YOUR PERSONAL INFORMATION
If at any time you believe the personal information we hold about you is incorrect or incomplete please notify our North Adelaide or Glenelg rooms as soon as possible to make the necessary changes.
You have the right to access your records but we reserve the right to charge an administration fee for searching for and copying records. This fee is an out-of-pocket expense to you and not claimable through Medicare or your Private Health Fund.
OVERSEAS DISCLOSURE
We will only disclose your personal information to an overseas recipient with your prior written permission. Correspondence would be to any practice or individual who may assist us in providing you with the best possible eye care.
UPDATES TO THIS POLICY
This Policy will be reviewed from time to time to take into account new laws and technology, along with changes to our operations and other necessary developments. Updates will be publicised on the Practice’s website.
